Background of the Study

Nurses play a crucial role in healthcare delivery, yet their involvement in hospital management decision-making has historically been limited. Effective healthcare management requires input from all stakeholders, including nurses, who are the frontline caregivers and have firsthand knowledge of patient care challenges (Adebayo & Musa, 2024). Studies have shown that hospitals where nurses actively participate in decision-making tend to have better patient outcomes, improved work environments, and higher staff morale (Ogunleye & Bello, 2023).

At Federal Medical Centre, Birnin Kebbi, nurses are expected to contribute to healthcare policies, resource allocation, and quality improvement initiatives. However, hierarchical hospital structures and administrative barriers often exclude them from key decision-making processes (Ibrahim & Adekunle, 2024). Research suggests that limited nurse participation in management leads to dissatisfaction, decreased productivity, and weakened interprofessional collaboration (WHO, 2023).

Despite global advocacy for shared governance in healthcare, there is a gap in empirical studies evaluating the extent of nurses' involvement in decision-making in Nigerian hospitals. Understanding the factors influencing nurses’ participation in hospital management at Federal Medical Centre, Birnin Kebbi, is crucial for promoting inclusive and effective healthcare governance.

Statement of the Problem

The exclusion of nurses from hospital management decisions remains a critical issue affecting healthcare delivery. While nurses constitute the largest proportion of hospital staff, their input in administrative and policy-making processes is often overlooked (Maslach & Leiter, 2024). Research indicates that limited involvement in decision-making negatively affects job satisfaction, professional growth, and healthcare service efficiency (Okeke & Yusuf, 2023).

At Federal Medical Centre, Birnin Kebbi, it is unclear to what extent nurses are involved in strategic decision-making processes. Factors such as organizational hierarchy, lack of leadership training, and administrative resistance may hinder their participation (Ibrahim & Musa, 2023). This study seeks to evaluate the current level of nurses’ involvement in hospital management and identify strategies for promoting their participation in decision-making processes.

Objectives of the Study

1. To assess the extent of nurses’ participation in hospital management decision-making at Federal Medical Centre, Birnin Kebbi.

2. To examine the barriers limiting nurses' involvement in management decisions.

3. To evaluate the impact of nurses' participation in decision-making on hospital efficiency and patient care.

Research Questions

1. To what extent are nurses involved in hospital management decision-making at Federal Medical Centre, Birnin Kebbi?

2. What are the key barriers to nurses' participation in hospital management?

3. How does nurses’ involvement in decision-making impact hospital efficiency and patient care?

Research Hypotheses

1. Nurses have limited participation in hospital management decision-making.

2. Organizational hierarchy and administrative barriers significantly hinder nurses’ involvement in decision-making.

3. Increased nurse participation in management decisions improves hospital efficiency and patient care outcomes.

Scope and Limitations of the Study

This study focuses on nurses at Federal Medical Centre, Birnin Kebbi, and their participation in hospital management decision-making. It does not include other healthcare professionals or hospitals outside the study area. Limitations may include reluctance of hospital administrators to disclose management practices and potential bias in nurses’ responses.
